Surveys and data improvements Merged

Add surveys that staff, families, or residents could fill out to choose content for residents, and improve benchmark data for areas like agitation, therapy needs, and loneliness, staff satisfaction, etc for research purposes. Would love a few more data points for content with levels and areas of cognition.
