Custom work request templates

Some communities have rooms that are available for residents or family members to reserve. In order for the maintenance team to know how to properly prepare the furniture in the room for that event they are currently filling out a paper form to understand table layout and additional needs for the room. They would like the ability to manage this in LifeLoop along with the remainder of their normal work requests.

Requirements:

• Customizable room request form that can be made available for residents, connections, and staff

• Ability to include images in the form to provide pre-set room layout options

I've attached examples of the forms the customer uses now as examples.
